Website homepage audience feedback questionnaire (blank)

This is the questionnaire i will use too assess what is good about the mock up of the home page for the magazines website and what needs improvement about it. I intend to give the questionnaire to 3 boys and 3 girls. This is less than the number of questionnaires given for the front cover, contents page, and double page spread because the website homepage is less important than them.The questionnaires will then be filled in by people of similar age rang (15-25) and the results will be posted.
